Students who will be entering Kindergarten need a physical and must be up to date on the required vaccinations.  If you have any questions, please feel free to contact Mrs. Terwilliger in the Elementary Health Office.

Please see the attached letter with guidelines on when you should or should not send your child to school when he or she is ill.

When To Keep A Child Home With Illness

This is the new Health Examination Form or Physical Form that is now required by NYSED.  After the 2018-2019 school year, this is the only physical form that we can accept.  If a physical is recorded on any other form, we will be unable to accept it and it must be resubmitted on the proper form.  All Physicians and Mid-Level Providers have been made aware of this change.  If you have any questions, please feel free to contact the Health Office at 607-525-6301.
